ValueCare - Part A

Healthy aging, along with independent living, has become a key challenge for Europe as countries experience growth in the number of older persons in their population. Several international organizations have stressed the importance of independence, participation, and autonomy for older people to maintain their health and, consequently, ensure their quality of life.

Project ValueCare is a Horizon 2020 EU project (Project ValueCare - Value-based methodology for integrated care supported by ICT ) designed to deliver efficient, outcome-based integrated care (both health and social) to older people facing cognitive impairment, frailty, and multiple chronic health conditions, with the aim of improving their quality of life and that of their families, as well as enhancing the sustainability of health and social care systems in Europe.

The project's vision of integrated, value-based care will be supported by a robust, secure, and scalable digital solution that will undergo testing and evaluation in 7 large-scale pilots across Europe, following a sound methodology developed by the project partners in collaboration with end-users. Project ValueCare aims to enhance efficiency in resource utilization and care coordination while ensuring user and policymaker trust regarding data access, protection, sharing, and standardization that can be replicated across the EU.

Project ValueCare has received funding from the European Union. Its primary objective is to provide personalized, integrated care services to older individuals, aiming to improve their health and social outcomes, as well as their care experience. Due to the project's scope, team members typically work remotely across various countries. However, they are currently gathered in Athens for an in-person meeting, offering a unique opportunity to discuss problems, share knowledge, and make important decisions about the project's future.
